The face at the end is the games creator John Romero, if you use noclip cheat and go inside the Icon of Sin on the last level of Doom 2 it is actually that head you are shooting at.

John Romero at the end, right? I remember finding him inside that huge wall at the end of a long hall. You can shoot it too, and it makes a face. That was such an awesome game, with its hardcore music and overall freakyness.
